Transfer of Peanut IgE Sensitization after Combined Pancreas-Kidney Transplant.
A 32-year-old female received a combined pancreas and kidney transplantation from an 11-year-old boy following his death from a fatal peanut ingestion. The recipient had no prior history of allergy to peanuts. Circulating specific IgE levels against peanut and recombinant peanut allergens (Arah1, Arah2) were measured in serum samples collected from the transplant recipient one month before transplantation in a stored specimen, and one, three and six months after transplantation. Skin tests were also performed one, three and six months following transplantation. The patient's pre-transplant peanut IgE levels were negative. At one month post-transplant, the patient had a positive 6mm skin test to peanut and had an elevated serum IgE blood level to peanut, Arah1, Arah2 of 0.79kU/L, 0.47kU/L and 0.25kU/L respectively. At three months, the skin test diameter and the IgE to peanut, Arah1, Arah2 decreased to 4mm, 0.69kU/L, 0.18kU/L and <0.1kU/L respectively. At six months post-transplant, the patient's skin test and IgE level to peanuts became negative, (Tables 1,2). At that time, the patient underwent an oral peanut food challenge without evidence of a clinical reaction thus excluding a persistent peanut allergy. The patient received a total goal dose of seven grams of protein without immediate or delayed hypersensitivity. The patient has since reintroduced peanuts into her diet. The donor's other organs were tracked and the other recipient's reactions to peanuts were investigated. The liver recipient developed peanut anaphylaxis, while the solitary kidney recipient remained unsensitized to peanut.

History of Allergy: History of food allergy.
"In this chapter we will first consider whether there is real evidence on the basis of literature for early descriptions in antiquity of pathogenic reactions after food intake that could be comparable to allergy, for instance in the scriptures of Hippocrates or Lucretius. On this topic we are skeptical, which is in agreement with the medical historian Hans Schadewaldt. We also assert that it is unlikely that King Richard III was the first food-allergic individual in medical literature. Most probably it was not a well-planned poisoning ('allergy') with strawberries, but rather a birth defect ('... his harm was ever such since his birth') that allowed the Lord Protector to bring Mylord of Ely to the scaffold in the Tower, as we can read in The History of King Richard III by Thomas More (1478-1535; published by his son-in-law, Rastell, in 1557). In 1912, the American pediatrician Oscar Menderson Schloss (1882-1952) was probably the first to describe scratch tests in the diagnosis of food allergy. Milestones in the practical diagnosis of food allergy are further discussed, including scratch tests, intradermal tests, modified prick tests and prick-to-prick tests. False-negative results can be attributed to the phenomenon of a 'catamnestic reaction' according to Max Werner (1911-1987), or to the fermentative degradation of food products. Prior to the discovery of immunoglobulin E, which marked a turning point in allergy diagnosis, and the introduction of the radioallergosorbent test in 1967, several more or less reliable techniques were used in the diagnosis of food allergy, such as pulse rate increase after food intake according to Coca, the leukopenic index, drop in basophils or drastic platelet decrease. The 'leukocytotoxic test' (Bryan's test), today called the 'ALCAT' test, shows no scientific evidence. The double-blind placebo-controlled food challenge test remains the gold standard in the diagnosis of food allergy. For the future, component-resolved diagnostics with the use of recombinant molecular allergens or chip arrays, such as the ISAC technique, hold a lot of promise. With regard to the clinical situation, a subjective selection is given, touching on the pollen-associated food allergies ('birch-mugwort-celery-spice syndrome'), as well as the new phenomenon of lethal food allergies that have appeared since the 1980s. Finally, rare ways of elicitation of a 'derivative allergy', first described by Erich Fuchs (1921-2008), for example by kissing, as well as 'oral allergy syndrome' and oral hyposensitization are considered."

Skin testing versus serum-specific IgE testing: which is better for diagnosing aeroallergen sensitization and predicting clinical allergy?
An accurate diagnosis of aeroallergen sensitization is pivotal to clinical practice and research. Given the recent technological advances in analyzing serum allergen-specific IgE, the question of which testing method, skin or serum testing, is superior in diagnosing allergic sensitization must be readdressed, as well as their value in predicting clinical disease. This review article provides a detailed summary of recent studies addressing these questions. Conclusively, most studies show substantial discordance between serum-specific IgE and skin testing results, suggesting that the two testing methods compliment each other and cannot be used interchangeably. On average, using only one testing method may misdiagnose every fourth allergically sensitized patient as non-sensitized. In addition, depending on the allergen tested, skin prick testing and serum-specific IgE testing appear to be the methods of choice in predicting outcomes of experimental allergen challenge, while intradermal testing is less contributory.

The molecular basis of peanut allergy.
Peanut allergens can trigger a potent and sometimes dangerous immune response in an increasing number of people. The molecular structures of these allergens form the basis for understanding this response. This review describes the currently known peanut allergen structures and discusses how modifications both enzymatic and non-enzymatic affect digestion, innate immune recognition, and IgE interactions. The allergen structures help explain cross-reactivity among allergens from different sources, which is useful in improving patient diagnostics. Surprisingly, it was recently noted that similar short peptide sequences among unrelated peanut allergens could also be a source of cross-reactivity. The molecular features of peanut allergens continue to inform predictions and provide new research directions in the study of allergic disease

The importance of hapten-protein complex formation in the development of drug allergy.
Drug allergy is an adverse drug reaction that is immune-mediated. Immune activation can occur when drugs or haptens bind covalently to proteins and then act as antigens. The purpose of this review is to summarize the recent data on the formation of hapten-protein complexes and to assess the importance of these complexes in the generation of drug allergy. The formation of hapten-protein complexes by drugs and their reactive metabolites has largely been investigated using model proteins such as human serum albumin. Precise identification of the structure of the hapten and the resulting modified residue(s) in the protein has been undertaken for a small number of drugs, such as p-phenylenediamine, nevirapine, carbamazepine, beta-lactams and abacavir. Some progress has also been made in identifying hapten-protein complexes in the serum of patients with allergy. Drug-specific T cells have been isolated from different patients with allergy. Formation of hapten-protein complexes, their processing and antigen presentation have been implicated in the development of drug allergy to p-phenylenediamine, sulfonamides and beta-lactams. However, evidence also supports the pi mechanism of immune activation wherein drugs interact directly with immune receptors. Thus, multiple mechanisms of immune activation may occur for the same drug

Allergic reactions to antibiotics in children.
This review analyzed recent findings on antibiotic hypersensitivity reactions in children focusing on betalactams, with regard to clinical entities, antibiotics involved and diagnostic methods. Betalactams are the most frequent cause of antibiotic hypersensitivity, more specifically amoxicillin alone or with clavulanic acid, with selective reactions to clavulanic acid also recently reported. Cephalosporins are the second most frequent group involved, especially in countries with high consumption. Other antibiotics such as sulphamides and macrolides although involved are less common. There are two types of reactions, immediate and nonimmediate, the latter being more frequent. Diagnosis is complex and is confirmed in less than 10% of children evaluated, twice as often in immediate than in nonimmediate reactions. Clinical history is often unreliable. Regarding other methods, skin testing and in-vitro methods can be useful for immediate reactions; however, most nonimmediate reactions need drug provocation tests for diagnosis. There are different degrees of cross-reactivity between penicillin and cephalosporins, with the side-chain being critical for determination.

Constipation and Cow's Milk Allergy: A Review of the Literature.
The causal association between cow's milk allergy (CMA) and constipation is not well established. Some guidelines describe constipation as a possible symptom of CMA, while others do not mention it. We conducted a literature review and found 10 prospective clinical trials. In all of them, an oral food challenge was performed, and 2 of them were randomized. These studies reported that a cow's milk (CM) protein-free diet has a beneficial effect on constipation, with a rate of successful outcomes ranging from 28 to 78%. The hypothetic pathogenic mechanism lies in increased anal pressure at rest, probably caused by allergic inflammation of the internal sphincter area due to mucosal eosinophil and mast cell infiltration. Eighty percent of patients reach tolerance within 1 year after the diagnosis of CMA-related constipation. We believe that a CM-free diet for 2-4 weeks should be proposed for children with chronic functional constipation, even if it is not severe or resistant to laxatives.

Allergy to formaldehyde: basophil histamine-release test is useful for diagnosis.
a case of formaldehyde-induced urticaria with a positive test result for serum IgE antibody against this substance. Formaldehyde's slow protein-binding property may explain why basophil histamine-release tests using fresh formaldehyde solutions are not diagnostic, whereas the tests are useful if formaldehyde that had been stored with albumin is used.

Non-IgE-mediated gastrointestinal food hypersensitivity syndrome in adults.
A report on a series of adult patients who presented with reproducible acute food-specific reactions manifested purely by gastrointestinal symptoms. Tests for food-specific IgE were negative. These reactions have some features in common with infantile food protein-induced enterocolitis syndrome (FPIES), a noneIgE-mediated gastrointestinal food hypersensitivity syndrome

that is well described in the pediatric literature. One similar case has been reported in an adult. The

observations on the clinical phenotype and demographics of noneIgE-mediated gastrointestinal food hypersensitivity syndrome in adult patients are presented.

Consecutive patients who presented with the following features of food hypersensitivity:

 Acute severe abdominal pain, repetitive vomiting, and/odiarrhea

 Reproducible on ingestion of specific foods on at least separate occasions

 Symptoms isolated to the gastrointestinal tract with theabsence of cutaneous, respiratory, or cardiovascular signs or symptoms of allergy

 Symptom resolution with avoidance of incriminated food

 Absence of food-specific IgE on skin prick tests and/orserology

 No other cause for symptoms identified

 Ag >18 years

Delayed anaphylaxis to red meat masquerading as idiopathic anaphylaxis.
Anaphylaxis is traditionally recognized as a rapidly developing combination of symptoms that often includes hives and hypotension or respiratory symptoms. Furthermore, when a specific cause is identified, exposure to this cause is usually noted to have occurred within minutes to 2 hours before the onset of symptoms. This case is of a 79-year-old woman who developed a severe episode of anaphylaxis 3 hours after eating pork. Before 2012, she had not experienced any symptoms after ingestion of meat products. Delayed anaphylaxis to mammalian meat has many contrasting features to immediate food-induced anaphylaxis. The relevant IgE antibody is specific for the oligosaccharide galactose-alpha-1,3-galactose, a blood group substance of nonprimate mammals. There is evidence from Australia, Sweden, and the United States that the primary cause of this IgE antibody response is tick bites. These bites characteristically itch for 10 days or more. Diagnosis can be made by the presence of specific IgE to beef, pork, lamb, and milk, and the lack of IgE to chicken, turkey, and fish. Skin prick tests (but not intradermal tests) generally are negative. Management of these cases, now common across the southeastern United States, consists of education combined with avoidance of both ingestion of red meat and further tick bites

Modified oral food challenge used with sensitization biomarkers provides more real-life clinical thresholds for peanut allergy.
This study sought to use a modified oral food challenge regimen that might determine threshold levels for peanut allergy mimicking a more real-life exposure and to correlate the eliciting dose (ED) and severity of clinical reaction in children with peanut allergy with B-cell, T-cell, and effector cell markers. A modified food challenge procedure with doses scheduled 2 hours apart was used in 63 children with peanut allergy. All children received a maximum of 8 semi-log increasing titration steps of roasted peanuts ranging from 3 to 4500 mg of peanut protein until objective allergic reactions occurred. Forty-five of 63 patients showed objective symptoms after greater than 30 minutes, with a median latency of clinical reaction of 55 minutes. By using a log-normal dose-distribution model, the ED5 was calculated to be 1.95 mg of peanut protein. The ED was significantly and inversely correlated with peanut- and Ara h 2-specific IgE levels, skin prick test responses, basophil activation, and TH2 cytokine production by PBMCs. Symptom severity did not correlate with any of the markers or the ED.

Eosinophilic bronchitis caused by styrene. A 55-year-old man who had been working as a panel beater for 22 years. Over the past 10 years he had developed chronic cough and dyspnea, which became more intense at the end of the work shift and at night; he remained symptom-free during holidays. He handled metal sheets and putty containing styrene as a hardener.

Occupational allergy in a holm oak pruner.
Occupational allergy in a holm oak pruner. He presented with episodes of ocular pruritus, tearing, conjunctival hyperemia, and chemosis, all of which are associated with the pruning of holm oak branches. Prick testing with holm oak wood dust was positive. IgE immunoblotting of holm oak wood extract revealed proteins of approximately 50 kDa capable of binding the patient’s IgE. Conjunctival challenge was positive.

Sensitization to Casuarina equisetifolia and Pinus spp pollen in patients with allergic rhinitis and asthma in Mexico City.
Pollinosis studies at Mexico City have found a considerable amount of Casuarina equisetifolia and Pinus spp pollen, its sensitization frequency is unknown. In Mexico, some allergens are not considered related to asthma or allergic rhinitis, even though reports in other countries have been demonstrated their relevance as aeroallergens. A transversal, descriptive trial was done at Hospital General de Mexico. 142 patients, 44 children (64% males) and 98 adults (73% females) were assessed. 8 (18.18%) children and 35 (35.7%) adults had a positive skin prick test to Casuarina equisetifolia. None of the patients included in the study had a positive skin prick test to Pinus spp.

Cow's milk allergy (CMA) in children: identification of allergologic tests predictive of food allergy.
Oral food challenge (OFC) is still considered the gold standard for diagnosis of food allergy (FA). Skin prick test (SPT) and specific IgE (sIgE) tests are very useful but limited in their predictive accuracy. End point test (EPT) has been recently considered to determine the starting dose to induce oral desensitization. Allergometric tests combined may discriminate children at higher risk of reactions during OFC. We considered 94 children referred to our Allergy and Immunology Pediatric Department between January 2009 and December 2011 with CMA. Cutaneous allergometric skin tests (SPT and EPT) were periodically performed on all 94 children with CMA; sIgE levels against cow's milk proteins (CMP) alpha-lactalbumin, beta-lactoglobulin and casein were periodically evaluated through blood samples every 6-12 months. During the period of the study, 26/94 (27.6%) children underwent more than once OFC. We collected 135 OFC compared with clinical presentation: 49/135 (36.2%) OFC were performed shortly after the onset of symptoms directly related to spontaneous intake of milk, to confirm suspicion of FA; 86/135 (63.7%) OFC were performed to evaluate the acquisition of tolerance. Of these, 52/86 (60.4%) OFC resulted positive, 34/86 (39.5%) were negative. The 3D EPT has the best ratio sensitivity (SE) / positive predictive value (PPV), SE 83%, specificity (SP) 58.3%, PPV 89.3%, negative predictive value (NPV) 45.1%. EPT 6D and 7D have the best PPV (100%) with a low NPV (respectively 22.2% and 21.2%). We obtained that a mean fresh milk wheal diameter >/= 12 mm was predictive of 97% OFC, but only 32/101 (31.6%) allergic children presented this value. The tests with a wheal diameter /= 12 mm wheal and 4 in the group between 6-11 mm). We also found that 95% of children with 4D EPT wheal diameter < 6 mm resulted tolerant. This cut off could be useful to decide which children have a lower risk of reactions during the OFC. EPT is more useful than SPT especially for children < 1 year of age being a less operator dependent test, and it could be helpful to discriminate between children with the highest risk to develop anaphylaxis following an OFC (>/= 5D positive EPT) and children with lowest risk (> 2D positive EPT), but it can't replace OFC, that currently remains the gold standard in the diagnosis of FA. We also underline that in allergic children younger than 9 months old, the values of SPT with fresh milk is much lower than in older children, so that it's better to separate this group of age when we try to predict the evolution of OFC through the evaluation with EPT. A validation of such results in a prospective study could maybe be useful to confirm the outcome of our data in the predictivity of OFC

Standardized testing with chlorhexidine in perioperative allergy.
In patients investigated for suspected perioperative allergic reactions, 9.6% were diagnosed with allergy to chlorhexidine. Using the authors definition of chlorhexidine allergy, the highest combined estimated sensitivity and specificity was found for specific IgE and SPT.

The high molecular weight glutenin subunit Bx7 allergen from wheat contains repetitive IgE epitopes.
This study characterized new wheat allergens using an IgE discovery approach and to investigate their IgE epitopes. A cDNA expression library was constructed and screened with IgE antibodies from wheat food allergic patients. IgE-reactive cDNA clones coding for portions of high molecular weight (HMW) glutenin subunits were identified. The major IgE-reactive areas of HMW glutenins were located in the repetitive regions of the protein and could show that two independent IgE-reactive fragments from HMW Bx7 contained repetitive IgE epitopes. Recombinant HMW Bx7 may be included into the panel of allergens for component-resolved diagnosis of wheat food allergy.

Immunoproteomics of processed beef proteins reveal novel galactose-alpha-1,3-galactose containing allergens.
We show that the alpha-Gal epitope is commonly present in IgE-reactive beef proteins recognized by meat allergic patients. Seven novel alpha-Gal containing IgE binding proteins were identified, of which four were stable to heat treatment. Thus the allergenicity of red meat proteins is preserved even upon different thermal cooking.

EAACI Food Allergy and Anaphylaxis Guidelines: diagnosis and management of food allergy.
Food allergy can result in considerable morbidity, impact negatively on quality of life, and prove costly in terms of medical care. These guidelines have been prepared by the European Academy of Allergy and Clinical Immunology's (EAACI) Guidelines for Food Allergy and Anaphylaxis Group, building on previous EAACI position papers on adverse reaction to foods and three recent systematic reviews on the epidemiology, diagnosis, and management of food allergy, and provide evidence-based recommendations for the diagnosis and management of food allergy. While the primary audience is allergists, this document is relevant for all other healthcare professionals, including primary care physicians, and pediatric and adult specialists, dieticians, pharmacists and paramedics. Our current understanding of the manifestations of food allergy, the role of diagnostic tests, and the effective management of patients of all ages with food allergy is presented. The acute management of non-life-threatening reactions is covered in these guidelines, but for guidance on the emergency management of anaphylaxis, readers are referred to the related EAACI Anaphylaxis Guidelines

The Prevalence of Serum Specific IgE to Superantigens in Asthma and Allergic Rhinitis Patients.
The present study suggested that IgE responses to staphylococcal superantigens are prevalent in the sera of both allergic asthma with rhinitis (BAwAR and allergic rhinitis alone (AR) patients. This may contribute to an augmented IgE response to indoor allergens and eosinophilic inflammation.

Clinical characteristics of gastroallergic anisakiasis and diagnostic implications of immunologic tests.
Recent studies have used the term 'gastroallergic anisakiasis' to describe incidental gastrointestinal infection with Anisakis spp. Larvae, proposed as a causative agent of food hypersensitivity. However, it is unknown whether this condition represents an independent disease entity distinguishable from acute gastric anisakiasis. To better understand the role of the allergic response in Anisakis infections this study examined the clinical and immunological implications of Anisakis-specific IgE. Seventeen case subjects and 135 controls were included. The case subjects had experienced gastrointestinal symptoms after raw seafood ingestion, along with additional mucocutaneous, respiratory, or multisystemic symptoms. Case subjects were significantly sensitized to Anisakis excretory-secretory product and crude extract compared with controls (76.5% vs 19.3%, P<0.001, and 88.2% vs 30.3%, P<0.001, respectively). Anisakis-specific serum IgE titers were also significantly higher in case subjects than in controls. Both the results of skin prick tests and elevated Anisakis-specific IgE titers (>17.5 kU/L) were found to be reliable indicators for the diagnosis of gastroallergic anisakiasis. The study concludes that among patients presenting acute gastric anisakiasis-like symptoms, a diagnosis of gastroallergic anisakiasis may be strongly supported by a high Anisakis-specific IgE titer.

Clinical aspects of 41 cases performed challenge test with immediate-type wheat allergy due to hydrolyzed wheat protein
Immediate-type wheat allergy due to hydrolyzed wheat protein (HWP-IWA) supplemented soap has been a serious social issue. This study investigated the significance of challenge test and other tests not only to diagnose HWP-IWA but to know the symptoms of each patients. Thirty nine of 41 cases were challenge test positive. In two patients who showed positive skin prick test but negative for challenge test, titer of specific IgE antibody and/or basophil histamine release test against Glupearl 19S® revealed positive reaction.

Pholcodine consumption and immunoglobulin E-sensitization in atopics from Australia, Korea, and Japan.
Accumulating data indicates that pholcodine (PHO)-consuming countries have higher sero-prevalences of immunoglobulin E (IgE)-antibodies to PHO and suxamethonium (SUX) and increased frequencies of IgE-mediated anaphylaxis to neuromuscular blocking agents (NMBAs) than nonconsuming. Withdrawing PHO-containing cough syrups resulted in a significant decrease of cases with anaphylaxis in Scandinavia. This study extended studies on PHO consumption and prevalence of IgE-sensitization to morphine (MOR), PHO, and SUX to countries representing high (Australia), and low (Korea and Japan), consumers, respectively. The prevalences of IgE-antibodies to PHO, MOR, and SUX were 10%, 8.6%, and 4.3%, respectively, in Australia. The corresponding figures for Japan were 0.8%, 0.8%, and 1.5%, and for Korea 1.0% to PHO and 0.5% to MOR and SUX. Of the SUX-positive sera, 100% were positive to PHO or MOR in Australia and 0% in Japan and Korea.
